VitalPath Health Program Service Agreement
This Service Agreement ("Agreement")is entered into by and between Cody Regional Health ("Provider") and the undersigned client ("Client"), collectively referred to as the "Parties."
1. Program Description: The VitalPath Health Program ("Program") provided by Cody Regional Health is designed to promote health and wellness through a structured series of educational modules, personal health assessments, and continuous support tailored to the individual needs of the Client.
2. Services Provided: The Provider agrees to deliver the following services under the Program:
- Access to the VitalPath online platform, which includes educational content, interactive tools, and tracking capabilities.
- Personalized health assessments to monitor progress.
- Regular consultations with healthcare professionals.
- Access to community support groups and health forums.
3. Client Obligations: The Client agrees to:
- Actively participate in the Program and complete all required activities and assessments.
- Provide accurate health information and update such information as necessary to maintain its accuracy.
- Adhere to the prescribed personal health recommendations and plans.
4. Payment Terms: The Client agrees to pay the Provider the sum of [insert amount] as a program fee. This fee is payable upon signing this Agreement and is non-refundable except as specified in Section 8 (Cancellation).
5. Term: This Agreement shall commence on the date signed and shall continue in effect for a period of [insert duration] unless terminated earlier in accordance with the terms of this Agreement.

7. Intellectual Property: The Provider retains all rights to the Program content and materials. The Client is granted a non-exclusive, non-transferable license to use the materials solely for personal health improvement as part of the Program.
8. Cancellation and Refunds: The Client may cancel their participation in the Program within [insert number of days] days prior to the start date for a full refund. Cancellations received after this period will not be eligible for a refund.
9. Limitation of Liability: The Provider is not responsible for the direct, indirect, incidental, or consequential damages resulting from the use of the services provided. The total liability under this Agreement shall not exceed the amount paid by the Client under this Agreement.
10. Governing Law: This Agreement shall be governed by the laws of the State of Wyoming, without regard to its conflict of law provisions.
11. Amendment: No amendment to this Agreement will be effective unless it is in writing and signed by both Parties.
12. Entire Agreement: This Agreement constitutes the entire agreement between the Parties and supersedes all prior agreements, whether written or oral, relating to the subject matter of this Agreement.
